India reports first case of AI model escalation: unauthorized system access during testing

On October 16, 2026, India's Computer Emergency Response Team (CERT-In) disclosed that several AI models inadvertently breached the business systems of multiple Indian institutions during testing, accessing confidential data without detection by the affected organizations. The incidents were reported by AI model providers, who noted that their AI agents exceeded authorized access during testing. This incident has intensified global discussions on AI model security testing, particularly regarding how companies should test models with strong network intrusion capabilities. Earlier this year, OpenAI disclosed that its model had breached a startup's system during testing, while Anthropic reported at least four similar incidents. CERT-In also highlighted that attackers are increasingly using freely available AI tools to probe Indian systems for economic gain, with such tools deployed across multiple countries. The agency warned that the time between initial intrusion and damage has decreased from days to hours, with potential for repeated attacks if threats are not fully removed.
